Why don’t I have double eyelids?

Double eyelids are formed by the attachments between the eyelid opening muscle and the eyelid skin. For people without double eyelids, these attachments are often weak or even absent.

To get double eyelids that are defined, symmetrical, and last for a long time or even permanently, then surgery is the best option. There are however a few methods you can use to get double eyelids without surgery, but these are only temporary fixes. Two of the most popular methods are:

1. Double eyelid tape or glue

2. Plexr plasma soft surgery

This is a non-surgical procedure (despite the name) that uses an applicator that emits a small electrical arc. When applied to the skin, the plasma arc stimulates its tightening. It is often used to tighten loose or droopy double eyelids. Effects can last from 12 up to 18 months.

This procedure, however, cannot be used to create double eyelids from single eyelids.

Types of double eyelid surgery

Double Eyelid Surgery

There are two options that my patients can choose from when it comes to double eyelid surgery. These are:

1. Non-incisional surgery (stitching)

In a non-incisional double eyelid surgery, sutures are stitched on each eyelid which attaches it to the deep eyelid muscle. This gives the appearance of a double eyelid crease.

A non-incisional surgery procedure lasts for just 15 to 35 minutes and causes some mild swelling and bruising after. My patients have a downtime of approximately 5 to 10 days after the procedure and they can expect their natural looking double eyelids to appear in 3-5 weeks.

The double eyelid crease created from this procedure will last for 1 to 2 years depending on several factors, such as the strength of the suture and the aftercare. This requires a greater amount of care and discipline compared to the other technique but has a faster recovery time and less scarring.

2. Incisional surgery (cutting)

For this procedure, I will first perform an incision on the skin of the eyelid to indicate the location of the double eyelid crease. After which I will permanently attach each eyelid skin with its respective eyelid muscle to create a double eyelid.

The great thing about this technique is that unlike the stitching technique, I can also correct other eyelid problems such as asymmetry, excess skin or fat, and droopy eyelids. I will be able to diagnose these other issues during the assessment and address these during the procedure. In addition to this, I can also enlarge the size of the eye or change its shape during the surgery.

The whole procedure takes about 40 to 60 minutes and some moderate swelling and bruising may occur. The downtime is longer compared to the stitching technique, which can last from 2 to 4 weeks depending on how you recover. Unlike the stitching technique where the double eyelid can disappear after a few years, the effects of incisional surgery is permanent.

Although the stitching method has the advantage of having less scarring and a quicker procedure and recovery time, in my opinion, the cutting method is the overall superior procedure for a couple of reasons:

  • The appearance of the double eyelid is tighter and more defined.
  • The double eyelid is permanent, and no additional procedures are needed to maintain the appearance.
  • The additional procedures that can be done during the surgery, such as removing loose skin and increasing the size of the eyes.

How the double eyelid surgery is done

Before the procedure, I will be using a marker to indicate the exact position of the incisions or stitches on your eyelids to ensure symmetry and precision. I will also show you a simulation of the final appearance of your eyelids after the procedure.

After this, you will be sedated, and local anesthesia will be applied on your eyelids. This is to minimize the pain and discomfort felt during the surgery.

Finally, the surgery itself is performed where depending on the method could last from 15 minutes to up to an hour. When the surgery is finished, you will feel some slight bruising and swelling on your eyelids. You will then be released and advised to rest and allow your body to recover.

  • If you have thick or heavy upper eyelids, you shouldn’t be undergoing non-incisional surgery since it will be harder for the stitches to stay in place. If the proper assessment was done beforehand, you should’ve been recommended the incisional surgery method.
  • If you constantly rub your eyes (consciously or not), the sutures can become loose and affect the overall appearance of the double eyelids. This weakens the attachments and your double eyelids might become asymmetrical and even disappear.
  • If you have eczema on your eyelids, this can cause irritation and might cause you to rub your eyes. Ensure that you have a full consultation before undergoing the procedure.

Scarring and other side effects of surgery

For non-incisional surgery, there is very little to no scarring at all. This is advantageous if you have a regular job and can’t afford to have a long downtime after the procedure. Since there isn’t much scarring, you can go back to work as soon as you are able.

For incisional surgery, however, scarring always occurs but will disappear after 3 to 6 months depending on how liberal you are in applying the scar gel. The scarring and long recovery time are a small price to pay for permanent, more defined and symmetrical appearance of double eyelids.

For both procedures, there will be mild to moderate bruising and swelling on the eyelids. These side effects should disappear within a few weeks though and shouldn’t be any cause for concern.

I advise my patients that they avoid strenuous exercise, such as long-distance running and weightlifting, during recovery. This is to ensure that your body gets enough rest and utilizes your energy to healing and rejuvenation. Painkillers and cold compresses will also help in reducing the swelling and bruising that naturally occurs after surgery.

Using a needle, the entry points of the threads are created, after which each thread is inserted carefully to lift the skin and fat tissue. After all the threads have been inserted, the excess threads are trimmed, and the procedure is finished.

The whole procedure should take around 25 to 35 minutes to finish. Depending on your pain tolerance, there may be very little to no downtime at all after the thread lift. Several patients of mine even had the procedure during their lunch breaks and come back to their jobs immediately after.

Expect some soreness and swelling around the entry points but these should disappear in a few days. The effects of a thread lift can last for up to 1 or 2 years and a follow up thread lift is recommended after.

Thread lifts cost around $1000 to $5000. The costs depend on several factors, which include:

  • The type of thread used
  • Your skin laxity
  • The doctor or clinic offering the thread lifts


Since thread lifts are minimally invasive procedures, there is a relatively fast recovery period. Swelling and bruising might occur directly after the procedure, but these usually go away after a few days. Pain killers can help to manage the aching but are not necessary.

  • Infection – the risk of infection is very low as long as proper aseptic technique is observed by the doctor during the thread lift.
  • Puckering and Rippling – rippling might occur on the skin if the threads are inserted unevenly, although it might also be caused by overtightening. In the case of over tightening, the issue should go away in a week.
  • Granuloma – this happens when the thread is placed too close to the skin surface and might stick out or irritate the skin. Consult your doctor or clinic if this happens so that corrective measures may be undertaken.

Melasma or ‘chloasma’ is a skin condition brought about by pigmentation that usually affects adults. It can be distinguished from other skin pigmentation flaws by its shape and colour. Brown and grey coloured irregularly shaped patches characterize melasma. It is prevalent among people with medium to dark skin tones or those whose skin tan easily.

Studies show that melasma is more prominent in people living in Asia and the Middle East.

Which parts of the body does Melasma affect?

Melasma affects certain parts of the body that are always exposed to the sun. Patches may form on the face, specifically on the cheeks, chin, forehead, upper lip, and nose. It can also be seen on the neck or forearms.

What causes Melasma?

There are several factors that can be attributed to the existence of melasma, but the precise cause of this condition remains unknown. The most common reasons for developing melasma are credited to hormones and ultraviolet (UV) rays from the sun, phototherapy, or sun-beds. On some occasions, cosmetic products or medications that irritate the skin may also trigger melasma.

Melasma generally affects women and is seen less in men. Women who undergo pregnancy or are taking contraceptive pills have a larger chance of getting melasma.
